Participation Agreement
I give permission for this child to attend and participate in all phases of the camp program, and I approve and certify that the applicant is capable of such an experience. I agree to pay the balance of the camp fees at least 14 days before the camp session. I understand that no refunds will be given if a child leaves early because of homesickness or for disruptive behavior as determined by the Camp Director. I authorize the Camp to take and use photographs and/or video of the camper for publicity purposes.
